GLP-1 medicines and HSE reimbursement

Dated Irish funding evidence for Saxenda, Wegovy and Mounjaro: managed access, NCPE assessment status and what an HSE funding decision means.

Compiled by John Fox. Sources checked 7 October 2026. This is an editorial source check, not a clinical review or a personal cover decision.

The answer before you budget

The HSE guidance checked on 7 October 2026 says Wegovy and Mounjaro are not covered by the medical card, Drugs Payment Scheme or Long-Term Illness Scheme. Saxenda has conditional managed-access reimbursement. Check the indication and current protocol with the prescriber; an NCPE assessment does not itself switch funding on.

Read the status for the correct medicine and indication

Wegovy, adult weight management: NCPE assessment 25024 was still ongoing when checked. Its latest process entry records the assessment restarting on 25 September 2026. That is an assessment milestone, not a reimbursement start date.

Mounjaro, weight management: NCPE assessment 24024 finished on 3 December 2025. Its recommendation was conditional on improved cost-effectiveness relative to existing treatment. This record does not establish that the HSE subsequently agreed funding or when an individual can access it.

Do not transfer a diabetes funding decision to weight management, or substitute one brand's decision for another because an ingredient is shared. Ask the clinician and pharmacist to check the exact product, indication and scheme together.

The adult Saxenda managed-access route

The linked adult protocol limits initial applications to ages 18–74, BMI at least 35, prediabetes and high cardiovascular risk. It also requires participation in non-medicine measures and review of exclusions. These are reimbursement criteria, not a self-assessment or a recommendation to take the medicine.

HSE: adult Saxenda Managed Access Protocol

The initiating prescriber applies to PCRS for individual approval before reimbursed prescribing. Initial support lasts 24 weeks. A continuation application checks response after 12 weeks at 3 mg/day, including at least 5% loss of initial weight. Approved support runs for a total of 24 months from the initial application; further treatment needs a new application.

The full protocol specifies laboratory evidence, cardiovascular measurements, exclusions and application timing. A BMI result from our quiz cannot confirm these requirements or approve reimbursement.

Children and adolescents have a separate pathway

The HSE introduced a separate Saxenda managed-access route for ages 12–17 from 1 February 2026. Applications require an appropriately registered specialist consultant and the adolescent protocol's own criteria. Adult thresholds and the adult quiz on this site should not be used to assess a child.

What would confirm a new funding route?

NCPE advises the HSE on effectiveness, value and budget impact. Its recommendation is one part of the HSE decision process. Approval to market a medicine, pharmacy stock, a pricing negotiation and public reimbursement are separate facts.

NCPE: interpreting recommendations

For a new announcement, check the HSE's effective date, funded indication, eligible population, application process and continuation rules before changing your budget. Our dated summary does not promise a decision date. Open the primary records below for any changes since our check.

  • Does the announcement name this medicine and this condition?
  • Has the HSE published when funding starts?
  • Is the application route operating?
  • Has the prescriber confirmed approval for this patient?

Could GLP-1 treatment suit you?

The quiz explains adult treatment criteria. It cannot approve HSE funding, insurance cover or tax relief. Three questions, about two minutes. See your BMI band against the criteria Irish providers publish.
